We will discuss the type of service or gathering you are considering, including things such as visitation, funeral or memorial services, church services, graveside services, cremation, burial, green burial, aquamation, or other wishes your family may have.
You do not need to make every decision before the arrangement conference.

It is helpful to think about possible dates, but please do not announce or confirm a service time until all locations and participants have been checked for availability.
If a church, cemetery, clergy member, or other location is involved, we need to make sure everyone's schedule works together before a date and time are finalized.
This can prevent having to change plans after family and friends have already been notified.

You do not need to arrive with everything figured out. We will guide you through the information we need and help you understand what happens next.

Do I need to complete this form before the arrangement conference?
No. The form is simply a way to give us a head start. Complete what you know and leave anything uncertain blank. We will review the information with you during arrangements.
Can I use this form to report a death?
No. If a death has occurred, please call Renaissance first at 919-866-1866. This form supports the arrangement process after you have spoken with us.
What should I bring to the arrangement conference?
If you have them available, it can be helpful to bring a photograph, clothing for the person who died, cemetery information, military discharge papers if applicable, and any notes or ideas you have about the service. If you do not have everything yet, that is okay.
What information will you need for the death certificate?
We will review identifying and family information with you, including details such as legal name, birth information, marital status, parents’ names, occupation, education, and other required information. You will have an opportunity to review and proof the information before it is finalized.
What if I do not know an answer on the form?
Leave it blank. You do not need to guess. We can help fill in missing information and verify details later.
Does submitting this form finalize the arrangements?
No. Submitting the form does not schedule services, authorize final arrangements, or replace the arrangement conference. It simply gives us information to begin preparing for our conversation.
Can we start planning even if hospice has just begun?
Yes. There is no “right” time to begin. Some families want to talk early, while others prefer to wait. You can reach out whenever it feels helpful.
